Fear & Hunger is a turn-based dungeon crawler set in an underground prison built on a site of ancient evil, where you choose one of four characters and descend to find a prisoner and escape. It's punishing without apology -- permadeath, extremely limited resources, a save system that requires finding and lighting specific candles, and enemies capable of killing you outright in the early floors. Combat is spatial and tactical, with a limb-targeting system that changes what enemies (and you) can do as pieces are lost. Made entirely by one Finnish developer, Miro Haverinen, in RPG Maker, and containing sexual violence, body horror, and sustained human degradation as structural content, not incidental shock -- this is genuinely not for everyone, and there's nothing wrong with that being a hard limit.

One of the most significant horror games I've played, not because of its extremity but because of what it commits to showing about human capacity for suffering that the commercial system structurally prevents. Read the content warnings seriously before buying -- this isn't shock for its own sake, but it is genuinely heavy, and knowing your own limits matters more here than with almost anything else on this site.
